1. Shift the focus from "whose fault is it" to "what do we do now"

When an incident occurs or a deficiency is discovered, the search for a scapegoat often begins immediately. But assigning blame doesn’t solve any problems in the long run; it only creates a culture of silence where employees are reluctant to report risks.

Here’s how to put this into practice: Whena risk or incident is brought up at the next staff meeting, consciously steer the conversation. Instead of asking, “Who forgot to close the gate?” or “Who was careless here?”, ask questions that encourage collaboration:
What circumstances led to this happening? 
How can we adjust our procedures to make it easier to do the right thing in the future?
What support or equipment do we need to solve this together?

By removing the blame, more people will feel comfortable speaking openly about safety, which is the foundation of a healthy work environment.

2. Establish a "3-to-1 rule" for improvement suggestions

It’s easy to point out things that aren’t working. Finding sustainable solutions, on the other hand, requires a little more thought. To tap into the collective intelligence in the workplace, you can introduce a simple but powerful rule:3-to-

3. Celebrate progress and highlight the actions taken

Nothing kills commitment to workplace safety issues faster than the feeling that “nothing ever happens anyway.” If employees point out problems during a safety inspection and the report then ends up gathering dust in a binder, they will soon stop speaking up about what they see.
